By completing this application form as legal guardian of the Vacation Bible School Participant listed above, herein known as the first party, I give permission for the said participant to attend the Riverside Baptist Church Vacation Bible School, June 9th through 13th, 2025. I also release Riverside Baptist Church, second party, from any claims or causes of action which may accrue while at the event mentioned above. Therefore, I hereby remise, release, acquit, satisfy, and forever discharge the said second party, of and from all and all manner of action and actions, cause and causes of action, suits, debts, dues, sums of money, accounts reckonings, bonds, bills, specialties, covenants, contracts, controversies, agreements, promises, variances, trespasses, damages, judgments, executions, claims and demands whatsoever, in law or equity, by direct claim or by indemnification, which said first party ever had, now has, or which any personal representative, successor, heir or assign of the said first party, hereafter can, shall or may have, against said second party, for upon or by reason of any matter, cause or thing whatsoever, from the beginning of the world to the day of these presents.

I also acknowledge by registering my child to attend Vacation Bible School, I grant Riverside Baptist Church permission to use photos/videos of my child for promotional purposes. These may include the church's website, newsletters, bulletin boards, emails, media, and special projects. However, my child’s name will not be used without my written permission, and the pictures/videos will be used exclusively for Riverside Baptist Church purposes. The Vacation Bible School leader may also take pictures or videos of activities that may include my child along with other children in the class, which may be shared.

For the safety of all children, we have specific sign-in and pick-up procedures. Children must be signed into and out of their classes by an Adult (18 and older). They cannot be signed in before 8:50 AM, and must be picked up by Noon. We appreciate your cooperation in adhering to these guidelines.

Submission of this form holds a space for your child. If your child cannot attend, please get in touch with Riverside Baptist Church to open the space for another child.

In order to provide a quality, safe experience for our children, those who miss the May 26, 2025 enrollment deadline will be accepted only if space is available. Availability will be determined the first morning of Vacation Bible School, June 9, 2025
